Case Management Nurse Manager

The role

The nurse case manager is the primary contact for a caseload of patients allocated to them and will conduct patient assessments by telephone this being the only source of patient contact.  Using your clinical knowledge and expertise in conjunction with listening, questioning and probing skills in order to interpret information obtained.

During telephone assessments the nurse case manager will develop patient management plans that encompass the physical and psychological needs of the patient, apply concepts of motivational interviewing and behavioural change techniques and incentives in order to affect improvement in patients’ outcomes.

Experience

- NMC Registered

- RGN

- Minimum 5 years post registration experience (preferably in primary and or community care)

- Computer literate (including Microsoft Excel and Word)

The nurse case manager must be able to constructively give/receive support within a team based environment and work within codes of practice and professional guidelines.  The nurse case manager must have excellent communication and interpersonal skills.  Evidence of strong customer service skills and well developed critical decision making skills are paramount.

Desirable

-  Evidence of contact centre/telephony working environment

- Evidence of working in a commercial environment.

Acute Trust Discharge Planner Nurse

Health Professionals are currently seeking an experienced nurse with current experience planning and organising the discharge of patients. Our client, a major International company requires a dedicated, experienced team member to join and already established team. You will manage the patients from the point of discharge and provide ongoing support encouraging them to maintain a balanced lifestyle and to prevent re-admission. You should have the ability to work autonomously as well as liaise with other Health Professionals seeking the best outcome for your patients.

• 2-5 years clinical practice experience , preferably in Acute Trust setting

• 2 years experience in: Case Management, Community Nursing, Acute Trust Discharge Planning

 

Education and Registration Requirements

 

• Registered Nurse currently registered with the NMC (Nursing & Midwifery Council)

• BSN desirable

 

Additional Information

• Ability to work with people in such a manner as to build high morale and group commitment to goals and objectives.

• Computer literacy and experience familiarity with Excel, MS Word, and web-based applications desired.
